Common Musculoskeletal Causes of Post-Crash Headaches
Cervicogenic Headaches

If these nerves become irritated by surrounding injured tissues, patients may develop sharp, burning, shooting, or electric-like pain beginning at the back of the head and traveling upward.
Neurological Causes
Concussion
Not every headache comes from the neck.
A concussion may occur even if you never lost consciousness or hit your head directly. Rapid acceleration and deceleration alone can produce enough force to injure the brain.
Headaches related to concussion may occur with:
- Brain fog
- Difficulty concentrating
- Memory problems
- Dizziness
- Balance difficulties

Vascular Causes: Rare but Important
Fortunately, serious vascular injuries are uncommon after a car crash. However, because they can be life-threatening, healthcare providers must always keep them in mind during the evaluation.
One example is an injury to one of the arteries supplying blood to the brain, called a cervical artery dissection. In rare cases, this can reduce blood flow or lead to a stroke.
Most patients with headaches after a collision do not have this type of injury, but recognizing the warning signs can be critically important.
The “5 D’s, 3 N’s, and an A”
Many healthcare providers remember possible warning signs of cervical arterial injury using the phrase “5 D’s, 3 N’s, and an A.”
These symptoms do not automatically mean someone is having a stroke or an arterial injury, but they should never be ignored after a neck injury.
The Five D’s
- Dizziness
- Diplopia (double vision)
- Dysarthria (slurred speech)
- Dysphagia (difficulty swallowing)
- Drop attacks (sudden unexplained falls)
The Three N’s
- Nausea
- Nystagmus (involuntary eye movements)
- Numbness, particularly affecting one side of the face or body
The A
- Ataxia, or difficulty with balance and coordination
If these symptoms develop after a collision, they warrant prompt medical evaluation.
Stroke Is Rare, But It Is an Important Differential Diagnosis
Stroke following a motor vehicle collision is uncommon, but it is one of the conditions healthcare providers must consider when evaluating certain headache patterns and neurological symptoms.
Seek emergency medical attention immediately if you develop:
- Sudden weakness of the face, arm, or leg
- Difficulty speaking or understanding speech
- Sudden vision loss or double vision
- Severe difficulty walking or maintaining balance
- Confusion or altered mental status
- Loss of consciousness
- A sudden, severe headache unlike anything you’ve experienced before
- Symptoms that rapidly worsen instead of gradually improving
These symptoms should never be dismissed as “just part of whiplash.”
Other Medical Conditions Can Also Cause Headaches
Not every headache occurring after a collision is caused by the collision itself.
Your healthcare provider may also consider other possibilities, including:
- Migraine disorders
- High blood pressure
- Sinus infections
- Eye strain or vision problems
- Viral illnesses
- Medication-related headaches
- Dehydration
- Less common neurological disorders
A thorough evaluation helps determine whether the headache is related to the crash or whether another medical condition may be contributing.
Why Documentation Matters
Proper documentation involves much more than recording that a patient has a headache.
A thorough medical record should document:
- When the headaches began
- Their location and severity
- What makes them better or worse
- Associated symptoms
- Neurological findings
- Functional limitations
- Changes over time
- The provider’s clinical reasoning regarding the likely diagnosis and any important conditions that have been ruled out
This information helps guide treatment, monitor recovery, and determine whether additional testing or referrals may be appropriate.
